Sutter Health Clinical Pharmacist Per Diem Evening Night Shift in Antioch, California

Description

Pharmacists ensure timely and accurate preparation and distribution of pharmaceutical products within Sutter Delta Medical Center. This position assures that the medication orders are consistent with current medical practice, F.D.A. and drug manufacture recommendations, current literature and/or other information sources. The Pharmacist resolves variations from the above standards with members of the Medical Staff in accordance with established policies and procedures. Pharmacists provide information and guidance in the proper administration of medications as necessary for health care providers, patients, and family. The Pharmacist will provide pharmaceutical care by performing medication-related distributive, clinical, and educational functions. Additional responsibilities include management of complex medication regimens for patients throughout the continuum of care.

Qualifications

Education/Licensures/Certifications:

  • Graduate from an accredited School of Pharmacy

  • Licensed Pharmacist - PHARMD

  • Completion of a ACPE accredited Pharmacy Practice residency

  • Board Certified Pharmacotherapy Specialists upon hire is preferred

Experience/Skills/Knowledge:

  • 3-5 years of prior acute care hospital pharmacy experience is preferred.

  • Comprehensive knowledge of current pharmacological/biopharmaceutical principles, medical terminology, pathologies, disease states, and related information for a medical management plan.

  • Knowledge of aseptic technique, and the preparation of sterile IV products and chemotherapeutic agents.

  • Possess excellent written and verbal communication skills.

  • Knowledge of pharmacy equipment and software.

  • Demonstrated ability to work with others.

  • Demonstrated ability to work in an independent and organizational manner.

  • Knowledge of federal and state pharmacy laws & regulations
