The Sutter Infusion & Pharmacy Services Pharmacist ensures the timely and accurate preparation and distribution of pharmaceutical products for the clinics and home infusion programs. Demonstrated knowledge is required in the following areas; IV hydration, pain management, infectious disease, chemotherapy, IV admixture, nutritional support, monthly and quarterly chart reviews, Medi Cal formulary, Medicare DMERC and restrictions, and third party reimbursement plans. This position assures that all medication orders are consistent with current medical practice, Board of Pharmacy Regulations, F.D.A. and drug manufacture recommendations, current literature and/or other information sources and drugs are prepared and distributed according to current accreditation guidelines. Periodically goes to patient homes to discuss and review drug therapy and the use of durable medical equipment. Attends meetings and works on committees as assigned by the Pharmacy Supervisor; these meetings may be at off grounds locations.
The Pharmacist provides information and guidance in the proper administration of medications on the request of other health care providers.
The Pharmacist delivers pharmaceutical care through the provision of clinical services, on call coverage for all patients including DME, drug information, preparation and distribution services that foster the safe and effective, as well as cost-effective, use of drugs in all patient age groups including geonates, pediatrics, adolescents, adults, and geriatrics.
Bachelor of Science or Doctor of Pharmacy degree from a recognized pharmacy school.
Current California pharmacy license with necessary continuing education to assure continued licensure.
Residency in Home Infusion or Hospital Pharmacy is desirable.
Previous experience as a Home Infusion or Infusion Clinic or Hospital Pharmacist is desirable.
Demonstrated leadership and team building skills are desired.
IV hydration, pain management, infectious disease, chemotherapy, IV admixture and nutritional support experience is desired.
Drug utilization review procedures, therapeutic drug monitoring, and knowledge of quality improvement, nutritional support, IV hydration, pain management, infectious disease, chemotherapy, IV admixture processes.
Knowledge in the preparation of monthly and quarterly pharmacy reports is required.
Computer knowledge is required.
PC Programs, pharmacokinetics is required.
Must be willing to act as a primary preceptor to pharmacy students.
Excellent interpersonal communication, presentation, and time management skills required.
Maintains a valid California Drivers License.
