Child/Adolescent Programs : Assists patients and families to gain access to needed mental health, medical, social, educational/vocational and other services.Assists patients and families in the implementation of planned discharge back to the community, building on patient’s gains and strengths.Maintains active working relationship with community agencies.Provides case management for the clinical therapists, schedules treatment team meetings and family sessions; Maintains tracking system for legal documents, assist nurses, physicians, and clinical therapist in obtaining consents and faxing information, assist in scheduling of outside patient appointments and works with staffing/unit clerk on transportation needs. Coordinates admissions/discharges with Service Line Managers; monitoring performance improvement processes related to patient discharges; obtaining new service authorizations and collaborating with Utilization reviewers for continued authorizations when needed.

Adult Residential Programs : Assists patients and families to identify and gain access to needed legal, mental health, medical, social, vocational/educational, housing, and other community services.Maintains active working relationships with State and community-based personnel.

Qualifications

Education:

Bachelor’s degree in Social Work or related field required.

Experience:

Prior work experience in a similar position, preferably in a healthcare setting. Experience with mentally ill populations in an inpatient or outpatient setting. Will consider applicant with limited psychiatric experience who has demonstrated interest in psychiatric mental health.

Knowledge:

Knowledge of community mental health services and resources. Knowledge of case management philosophy and functions, treatment modalities and evidence based practices for emotional/psychiatric disorders. Knowledge of managed care and the requirements of third party payers preferred. Familiarity with medical and mental health terminology desirable and general office procedures.

Specials Skills/Equipment:

Basic office equipment and computer skills are required; proficient in computer software programs Microsoft Word, Excel, PowerPoint, Good written and verbal communication skills required. Ability to work cooperatively with a multi-disciplinary team. Ability to function independently, prioritize and organize the efficient use of time. Ability to work with patients of all ages and diagnosis. Must have demonstrated organizational skills with attention to detail, follow-up and ability to manage caseload. Valid Driver’s License is required.

Physical Requirements:

Must meet the basic health requirements of Kahi Mohala’s physical examination including TB skin test or chest x-ray and drug screen. Must obtain and maintain current CPI (Crisis Prevention Intervention) certification. Sedentary work, occasional crouching, frequent walks between buildings. Frequent walking between units; bending, stooping, kneeling, pushing and ability to lift 20 pounds.
