Description

Incumbent will evaluate, establishes treatment plan, and provide physical therapy services upon physician referral for identified patients. The physical therapist perform services in an interdisciplinary environment with occupational therapy, speech pathology, physicians, nursing and other health care providers. All patient services are documented appropriately and updated to justify continued intervention. Incumbent provides continuum of care services to inpatient, outpatient, extended care unit, and home health patients. Incumbent will provide supervision to physical therapy assistants and aides, as allowed by law and regulations.

Qualifications

Bachelor's Completion of BA Degree in Physical Therapy. Required

Master's Completion of Master’s Degree in Physical Therapy. Preferred

Physical Therapist - PT Required

Basic Life Support Certification - BLS Required

Driver License, Valid and in State - DL NUMBER Required

Previous home health experience in physical disabilities and rehabilitation is desired. Preferred

One year acute or home health experience in past three years required. Required

PM or weekend shift required

May be required to work overtime and/or weekends to meet patient needs. Adjustments to shifts may apply based upon hospital needs
