Sutter Health Coordinator-Intake Full Time Day Shift in Oakland, California
Educates, supports and assesses clients referred to the Chemical Dependency Program. Recommends treatment and coordinates admission to treatment for clients.
Responds to inquiries regarding to need for chemical dependency treatment from prospective patients, families and both internal and external referral sources.
Utilizes criteria to conduct clinical assessments in order to determine level of client functioning, appropriateness of treatment and type of treatment services required.
Arranges for admission by coordinating financial, clinical and other pertinent information including insurance verification and treatment authorization.
Provide concise, relevant clinical information about clients to treatment team responsible for client care.
Master's Degree is required.
LCSW or MFT is required.
CADAC Certification is preferred.
- 2 years experience in Chemical Dependency is preferred.
