Sutter Health RN Heart Transplant Coordinator - Transplant, Heart Specialty Clinics (Full-Time, Day Shift) in Sacramento, California
The Heart Transplant Coordinator coordinates all services/systems required for an organized multi-disciplinary team approach to the evaluation, treatment and follow-up of the patient needing transplantation services. The Transplant coordinator will have on-call responsibilities to take organ donor calls, coordinate organ screening, matching and recovery process, arrange surgery preparations for the transplant recipient, and triage after hours patient medical calls for on-call transplant physician. The Transplant Coordinator will have an area of specialty designated (hearts or kidneys), however, will be cross-trained to take on-call responsibilities for all heart and kidney transplant patients.This includes evaluation, treatment and follow-up of the living kidney donor.In addition, cross-training may be required for Heart Specialty patients including: VAD (ventricular assist device), HF (heart failure), TAVR (transcatheter aortic valve replacement) ACHD (Adults with Congenital Heart Disease) and Cardiac Rehab. Population served includes adolescents, adults, and geriatrics.
I. Plans and delivers expert clinical care (both inpatient and outpatient) for the adult (renal and/or heart) transplant patient and living kidney donors.
Utilizes specialized clinical knowledge to: Assess, explain and direct patient care through the transplant process by functioning as a participant in direct patient team care. Anticipates patient problems based on the knowledge of patters of physiological, pathological and psychological responses in the adult patient participating in the transplant process.
Uses assessed information to formulate a comprehensive plan of care which uses patient centered goals and provides for continuity of both inpatient and outpatient care.
Collaborates with administrative/clinical staff and physicians to develop and implement treatment protocols and standards of care using a multidisciplinary approach.
Evaluates outcomes as a participant in direct patient care: Evaluates quality of patient care and nursing practices based on established standards and needs of the patient and family. Evaluates short-term and long-term responses to therapeutic intervention throughout all phases of the transplant process.
Collects and evaluates data pertinent to vascular organ donation and transplantation making recommendations for suitability of potential vascular organ living donor and/or transplant recipient based on program’s identified criteria for selection and protocols.
II. Provides and coordinates Living Kidney Donor/Recipient and Family Education and Participates in Staff Education.
Establishes and maintains criteria and standards for formulating transplant patient/family teaching plans: Identifies, plans, and implements interventions for the educational needs of the living kidney donor/transplant recipient and families using a multidisciplinary team approach. Designs and implements teaching strategies that assess learning readiness, demonstrates application of the adult teaching and learning theories and utilize hospital/community resources as appropriate.
Participates in presenting at conferences, seminars, and other educational offerings within and outside of Sutter Health to promote awareness of organ donation and transplantation.
III. Acts as Coordinator/Liaison/Consultant
Functions as a participant in direct patient care team to provide effective integration of the patient into the transplant services by coordinating the services throughout the transplant process.
Maintains an organized system to coordinate all aspects of transplantation surgery, which includes timely notification of team members, accurate communication with team, implementation of transplant protocols/procedures including those pertaining to the living kidney donor.
Develops collaborative interdisciplinary relationships and communicates to facilitate and support quality care of the transplant patients.
Serves as a resource/liaison for physicians, nursing staff and other members of the interdisciplinary staff.
Works in conjunction with the Public Relations Department to provide community agencies, including referring doctors and patient families, with information on transplantation and organ donation.
IV. Serves as an Evaluator/Researcher.
Evaluates the transplant system for quality, efficiency, safety and effectiveness of established standards and protocols.
Ensures that a transplant registry/database is accurate and up-to-date.
Actively participates in the review of transplant cases with appropriate committees/individuals to assist in the selection of Quality Service Indicators, auditing, analyzing and reporting within the established Quality Assessment Program.
Uses scientific methods to further nursing knowledge of transplant patients by participating in the development and implementation of clinical research projects that reflect a review of the current literature and are relevant to patient care issues.
V. Maintains documentation as required by hospital, state and federal regulatory bodies.
Documents patient/family assessments, plans, interventions, and the patient/family’s response to intervention in the medical record based on the direct patient care given.
Ensures that outpatient medical records are maintained on all transplant patients with accurate and complete information.
Completes required United Network for Organ Sharing correspondence and returns within working hours.
MINIMUM POSITION REQUIREMENTS
Graduation from an accredited school of registered nursing. Current California RN license. Bachelor’s Degree preferred or required within 3 years of hire; Masters preferred (or equivalent transplant clinical experience) and clinical research knowledge, preferably within area of transplant (i.e., hearts or kidneys). CCTC (Certified Clinical Transplant Coordinator) certification encouraged to obtain within 3 years once hired.
Recent patient care experience in transplantation, with transplant specialty experience preferred. Transplant coordination experience preferred. (Minimum of 2 years clinical transplant experience is preferred).
Knowledge of pathology, physiology, and immunology related to solid organ transplantation, specifically in the area of transplant specialty. Additional knowledge regarding secondary specialty is preferred. Knowledge of adult educational planning, implementation and evaluation. Knowledge of the United Network of Organ Sharing and applicable regulations and policies preferred.
· Special Skills/Equipment:
Excellent interpersonal and communication skills, both oral and written are necessary; excellent organizational and time management skills. Clinical research skills. Basic computer skills (access, enter and retrieve data; MS Word and Excel spreadsheets).
· Physical Requirements:
Moderate physical effort. Must have body mechanics knowledge to safely move/handle equipment and/or office supplies. The Transplant Coordinator is required to provide on-call 24 hour coverage.
