Sutter Health RN - Home Infusion Coordinator - SIPS in Sacramento, California
Under the direction of the Home Infusion Program Manager, the Home Infusion Liaison is also responsible for assisting in the hiring process; orientation of new employees, in-service training, and evaluation of qualified personnel for all disciplines involved in patient care infusion services. This position coordinates all interdisciplinary HIT Meetings and maintains knowledge specific to the needs of the unique patient populations. The Home Infusion Liaison provides up to 25% direct patient care, or as directed by the Home Infusion Program Manager. Home Infusion Program Manager may assign other managerial tasks and projects as necessary.
RN graduate from an accredited curriculum. Current California RN license, valid California Driver License with proof of current auto insurance; current BCLS CPR certification. CRNI preferred.
Experience typically received with a minimum of 2 yrs work in home infusion therapy in the past 5 years
Advanced understanding of home care infusion services, infusion clinical standards of care and practice; must have clinical knowledge and critical thinking ability to effectively plan and provide infusion patient care consistent with the standards and regulations of Title XXII, Medicare Conditions of Participation, INS Standards of Practice, JC home health accreditation requirements, QI outcomes/benchmarking, and infection control specific to infusion patients. Good understanding of reimbursement patterns for Medicare and other payers. Must have excellent organizational and supervisory skills to handle multiple level complex patient care issues.
Infusion, phlebotomy/venipuncture experienced and trained. CRNI and ONS chemotherapy trained is preferred.
Excellent written and verbal communication skills. Able to utilize a telephone for internal and external communications. Competent in care provision in area of professional licensure. Self-starter with a high degree of initiative, motivation, flexibility, energy and creativity. Ability to form harmonious working relationships with internal and external customers. Demonstrated leadership and negotiation skills and the ability to manage resources for infusion patient care in a cost effective manner. Excellent collaborative and problem solving skills with customers and management. Ability to exercise independent sound judgment in planning and providing patient care. Demonstrated ability in physical, psychosocial, and environmental assessment skills and in implementing plan of treatment. Excellent organizational skills. Access to a reliable automobile.
Must be able to meet company productivity standards as indicated by organization and manager. Maintain a positive and professional image and demonstrate self-directed learning to meet professional development and/or license requirements. Is responsible for maintaining all required licensure and certifications. At orientation and annually thereafter, must be able to meet Sutter Care at Home core competencies for position and area of specialty, as appropriate. Must have reliable transportation.
