Sutter Health Utilization Review Licensed Vocational Nurse (LVN) / Licensed Psychiatric Technician (LPT) (Full-time, Days) in Sacramento, California

Description

The Utilization Review/Discharge Planner (UR/DP) performs chart review to determine the appropriateness of care to ensure effective quality and cost-efficient outcomes, coordinates all aspects of patient care involving all members of the health care team to develop an integrated discharge plan. Uses effective interpersonal skills in managing the complexity of interactions involved with the position. Must be able to communicate in a timely manner to insurance case management providers to assure continued and discharge utilization management appeals to patient care needs.

Principal Accountabilities

  • Coordinates the discharge plan that reflects the after care needs of the patient per the physician's plan of care.

  • Monitors the plan of care and coordinates the delivery of services.

  • Participates in the collaborative treatment planning and active care of the patient.

  • Promotes integration of physician’s plan of care and the discharge plan.

  • Under the direction of physicians reviews and determines appropriateness of admission and level of care.

  • Timely provides or obtains authorizations, coordinates insurance benefits and utilization management to promote high quality outcomes and cost effective service delivery.

Qualifications

Education, including Licensure/ Certification/ Registration

Graduate of an accredited Psychiatric Technician Program/Vocational Nursing Program. Must possess and maintain California licensure as a Psychiatric Technician/Vocational Nurse.

Experience

Seasoned experience in psychiatric nursing, preferably in an inpatient setting is required. Experience working with all age populations is required. Demonstrates experience utilizing effective interpersonal communications, organizational and priority setting skills. Demonstrates ability to plan, organize and support implementation of a patient focused program in a healthcare setting.

Skills

The ability to work independently with a minimum of direction. Critical thinking skills necessary to provide utilization review/discharge planning services appropriate to patients with complex medical, emotional and social needs. Excellent interpersonal skills and time management skills. Effective oral and written communication skills.
