Qualifications

Registered Nurse - Primary Care Clinician

Education

RN graduate from an accredited curriculum. Bachelor’s degree in nursing or health related field preferred.

Licensure

Current California RN license; valid California driver’s license; current auto insurance; current BCLS/CPR certification.

Experience

One year of professional experience in an acute or similar setting required. One year of experience in a home health and/or hospice position preferred. Direct patient care experience within the past five years required.

Physical Therapist

Education

Graduate from an accredited physical therapy program. Masters in Physical Therapy or DPT preferred.

Licensure

Current California PT license; valid California driver’s license; current auto insurance; current BCLS/CPR certification.

Experience

One year of professional experience in an acute or similar outpatient setting required. One year of experience in a home health and/or hospice position preferred. Direct patient care experience within the past five years required.

Occupational Therapist

Education

OT graduate from accredited curriculum required.

Bachelor's Science, health care related area of study preferred.

Master's in Occupational Therapy preferred.

Licensure

Occupational Therapist - OT Required

Driver License, Valid and in State - DL NUMBER Required

Automobile Insurance - INSURANCE Required

Basic Life Support Certification - BLS Required

Experience

One year of professional experience required.

One year of experience in a home health and/or hospice position preferred.

Direct patient care experience within the past five years required.

LVN

Education

Graduate from an accredited vocational school of nursing or equivalent.

Licensure

Licensed Vocational Nurse - LVN

Current California Blood Withdrawal certification, or to be obtained within 30 days of hire date OR Certififed Phlebotomy Technician Level I - CPT I

Basic Life Support Certification - BLS

Automobile Insurance - INSURANCE

Driver License, Valid and in State - DL NUMBER

Experience

Minimum 1 year of professional experience required.

Direct patient care experience within the past five years required.

Home health or community health experience preferred.

Speech Therapist

Education

SLP graduate from an accredited curriculum. Master’s degree required for SLP.

Licensure

Current California SLP license, valid California Driver License with proof of current auto insurance, current BCLS CPR certification.

Experience

One year of professional experience strongly preferred.

One year of experience in a home health or hospice position preferred.
