Sutter Health Cardiovascular Nurse Practitioner/Clinical Educator - PD in Santa Rosa, California
The Cardiovascular/Cardiothoracic Nurse Practitioner/Clinical Educator has a pivotal role in ensuring the coordination and smooth running of the Cardiac Surgery service. By working independently and in collaboration with the surgeons, cardiology and pulmonology staff, this individual is responsible for coordinating multiple hospital and outpatient services for the cardiovascular patients and their families. The major role functions of the NP include clinical practice, consultation, education, research improving efficiency or decrease costs and leadership/professionalism.The incumbent has access to Protected Health Information as needed to accomplish the accountabilities as outlined in this job description. This includes access to the patient's medical record.
Collaborates with cardiovascular surgeons and nurses to facilitate the care of cardiovascular surgery patients throughout the continuum of care.
Works effectively in close collaboration with the cardiac surgeons, pre and post operative nurse educator, case managers, pharmacists, social workers, dietician and medical staff.
Assessment of the staff and patient education process and achievement of staff and patient education goals.
During the postoperative period, the NP provides information to the physicians regarding the follow-up care of these patients, including assessment, clinical testing, medical management and any wound complications based on standard procedures.
Provides patient education and discharge teaching.
Provides consistent follow-up and performs a comprehensive history and physical exam at surgeon’s request.
Provide critical care education and precept nurses orienting in the Intensive Care Unit.
Implements and evaluates standards of practice with special attention to cost effectiveness, caring, comfort, safety, and cultural diversity.
For the cardiovascular and cardiothoracic in-patients, the NP mentors staff nurses and charge nurses.
Works collaboratively with the nursing staff to develop or facilitate the development of a comprehensive nursing care plan especially with complicated or long-term patients.
Represent the department on hospital-wide committees as assigned.
Consults with the other Education providers concerning educational requirements as needed for our nursing staff. Ensures there is a formalized educational program, which is implemented and updated as needed to meet the current standards.
Provides individual and small group instruction, as well as, lectures, seminars, and conferences.
Maintains current knowledge and competency in the area of specialization and in professional practice through relevant conferences, workshops, preceptorships, and professional organization affiliations.
Contributes to the expansion of the body of knowledge in nursing within area of specialization through research and encourages staff to do the same.
Ensures there is a coordinated and effective discharge plan in place.
Demonstrates mastery in the ability to use all components of the nursing process including assessment, planning, implementing and evaluating care to meet the individual critical care and/or cardiovascular patient’s needs according to their age, culture and conditions.
Assesses patients, identifies obvious as well as subtle patient problems and/or abnormal assessments, anticipates changes in patient’s condition and appropriately reports and resolves.
Develops plan of care based on age specific assessment. Involves patient, family and interdisciplinary resources to develop and revise the plan of care.
Utilizes advanced interpersonal skills to facilitate problem resolution and collaborate and develop a plan with members of the health care team, patients and their families to ensure that patient’s needs are met.
Manages clinical situations with advanced skills, ensuring that the patient/family’s physical, psychosocial and cultural needs are met. Supervises patient care and delegates appropriate components based on staff competence and patient care needs in a manner that promotes efficiency, teamwork and optimal patient outcomes.
Administers medications and treatments according to hospital policy and procedure and accepted standards of age specific care and department standards.
Communicates effectively with patients, families and other health care providers.
Formulates and participates in strategies to maintain and improve patient care outcomes. Coordinates multidisciplinary care conferences as needed.
Reassesses and evaluates patient response to care and takes appropriate action to assure patient’s needs are being met.
Participates with patient/family and other members of the healthcare team in evaluation of patient outcomes.
Utilization Management Program for Cardiovascular Program
Supervises the clinical admission process of cardiovascular program to assure patients meet admission criteria and are placed in a timely fashion on the appropriate level of care.
Performs concurrent and retrospective utilization review of cardiovascular program to assure that the appropriate level of care is maintained throughout the patient’s hospital stay.
Follows up with insurance companies to provide information to justify continued patient stays
Acts as resource to case managers for coordination of complex discharge planning needs and referrals to appropriate support services.
Maintains a database of clinical information on all patients.
Participates in the identification, collection, and analysis of data to support Utilization Management (for example, activities affecting patient length of stay, such as avoidable days).
Partners with physicians for effective clinical management and utilization of resources for the patient population.
Identifies Utilization Management issues and takes action by working with medical staff or reports appropriately to the Medical Director or department, any issues identified.
Demonstrates leadership in handling daily activities on the unit.
Promotes collaborative/caring relationships.
Acts as a clinical resource and mentor to team members.
Supervises assigned staff members, based on knowledge of the person’s job description and scope of practice.
Maintains a safe work environment by adhering to hospital, departmental policies and procedures and regulatory requirements.

Assists in the development, research and revision of patient care standards of care and standards of practice, policies and procedures.
Assists with the implementation of new programs, systems and equipment.
Serves as a role model and as a resource to others regarding the efficient use of resources and the use of equipment/products.
Master's Degree in nursing from an accredited School of Nursing Required
Previous experience as an RN (generally 3-5 years). Required
Demonstrated clinical expertise in area of specialty for specific clinical areas required
Working knowledge of the cardiac surgery and cardiothoracic patient required
Previous experience with formal and informal cardiac patient education. Required
Registered Nurse - State Licensure - RN Required

Basic Life Support Certification - BLS Required
Advanced Cardiac Life Support - ACLS Required
In-depth physical assessment/management skills
